The Adult Medical Day Care program at Pleasant Valley offers health care
services to adults living in the surrounding communities who are functionally
impaired, but do not require 24 hour per day nursing home care. The Adult
Medical Day Care is for men and women, 18 years or older, who are physically
impaired or who are experiencing social isolation. The overall goal of the
program is to promote client wellness by providing interventions designed to
limit decline of their current level of functioning and by assisting them in
obtaining supportive services necessary to maintain their independence in the
community.
The Adult Medical Day Care program is open from 8:00 AM to 3:00 PM, Monday
through Friday (except legal holidays) and is open year round. Families are
encouraged to provide transportation, if possible. If none is available,
transportation will be coordinated through the facility or local
transportation sources.
