Purpose:
To address the patients' safety, physiological and psychological needs in a patient plan of care.
To address comprehensive quality patient and family centered care in the patient's place of
residence.
To provide home health services that promote the patient's value of life by providing those
services in the most appropriate home care program.
To provide support and care for persons in the final stages of life so they may live in comfort and
dignity surrounded by the presence of their family, friends, and care givers through the hospice
program.
The Hospice and Palliative Care Program is certified by the US Department of Health, Education and Welfare, and the New York State Department of Health. It is the only certified hospice program in Washington County. Services are provided in the patient's place of residence whether it be a home, nursing home, or at a hospital if admitted. Care is provided under the supervision of a physician and the multi-disciplinary team who meet three times a week to follow-up on care giving issues.
