In-Home Personal Care Assistance

Provides non medical personal care, housekeeper chore aide services, case management, information and referral services and supportive counseling to assist those 60 and over to remain living independently at home.

Funded under the States Expanded In Home Services For the Elderly (EISEO), this is a coordinated package of non-medical services and supports developed from an assessment to assist older adults who need help with Activities of Daily living (i.e. dressing, bathing, personal care) and instrumental Activities of Daily Living (i.e. shopping, cooking). In-home services and supports provide more than just assistance with tasks. All enrollees receive case management and, when appropriate, could also receive additional supports, including;

  • Social Adult Day Services.
  • Personal Emergency response Services (Medical Alert)
Consumer Directed Services

We also offer Consumer Directed In-Home services.  Eligibility requirements include those provided above, as well as:

  • Be capable of making choices regarding their In-Home services,
  • Be capable of hiring, training and managing in In-Home services worker,
  • Understand the impact of their choices; and assume responsibility for the results of these choices; or
  • Delegate authority to another who is capable of these tasks.
